[hs-equality] Chauvinism and PyCon

1. The Ada Initiative (http://adainitiative.org/) has standard codes of
conduct that conferences can adopt. I haven't had a chance to look them
over yet, but the resources are already out there.

2. The PyCon code of conduct described what to do in the case of a
violation, and included contact info for the organizers, although it was a
little vague as to the specific procedure for contacting them. Having an
easily accessible code of conduct that includes contact info and specific
steps for reporting inappropriate conduct seems like the right way to go.
