A liability release is worth exactly as much as you paid your lawyer to
write it, but here is the one we use:

mentioned in another thread they require everyone to sign one as they walk
in.
The Insurance page gives a bit more background, linking to some threads in
this list. Getting the right description of the space, that fits a box the
insurance company can deal with (by offering a policy, not turning you
away, or running away themselves), seems to be very important.
