[hackerspaces] Membership documents

We are in the process of writing our membership document and I was  
wondering if anyone could point to any existing hackerspace membership  
documents anywhere? I am referring to what people sign when they  
formally join the hackerspace and start paying dues, a legal document.  
Pumping Station One's Membership Manual is an excellent document, but  
that appears to be a set of guidelines, not something that gets signed.
